An Australian government review has determined that significant "failures" by the Israeli military led to a drone strike on an aid convoy in Gaza, resulting in the deaths of seven workers from World Central Kitchen on April 1. The casualties included charity workers from Australia, Canada, Poland, the UK, the US, and their Palestinian colleague.

The review, released on Friday, highlighted that the Israel Defense Forces did not adhere to established procedures, resulting in identification and decision making errors. Australian Foreign Minister Penny Wong stated that the government would seek full accountability for those responsible, including potential criminal charges. The military advocate general of Israel has yet to decide on further action.

Following international pressure, the IDF initiated an internal investigation, admitted the strike was a "serious failure," and dismissed senior officers. The charity team had been authorized by the Israeli military to transport aid supplies but was mistakenly targeted by drone operators who believed the vehicles were carrying Hamas gunmen.

WCK founder José Andrés accused the Israeli forces of systematically targeting aid workers. WCK called for an independent investigation, criticizing the IDF's use of deadly force without following protocols and rules of engagement.

In response to the attack, the Australian government appointed former defense force chief Mark Binskin to review the incident. The Australian government has committed to implementing all of Binskin's recommendations, which include urging Israel to enhance coordination with humanitarian organizations.

The IDF has reprimanded three commanders and indicated that those responsible for the attack may face criminal charges. Wong emphasized the need for transparency in the military advocate general's process and decisions. Since October 7, 2023, over 250 aid workers have been killed in Gaza, according to the United Nations.
